Q1. Are my responses anonymous, or can I be identified?
A: The invitation email provides a link with an embedded tracking ID. This is used for internal purposes only. Beyond this, this survey is confidential. Individual responses will not be disclosed to the public.
Q2: Can I print out the survey and fax or send my responses to you?
A: You will need to complete the survey online.
